ORA-15477: cannot communicate with the volume driver

What triggered the Error:

An attempt was made to communicate with the volume driver.

What should we do to fix it:

Check that the ASM volume driver is loaded. If so, check the alert log to identify the reason for failure and take necessary action to prevent such failures in the future.
