ORA-15044: ASM disk 'string' is incorrectly named

What triggered the Error:

Either the specified disk had its contents changed such that it no longer contained an ASM disk name in its header that matches the diskgroup information or its FAILGROUP information may have become inconsistent.

What should we do to fix it:

Drop the disk from the diskgroup.
